- Hip fracture = any fracture of the proximal femur within 5 cm distal to the lesser trochanter
- Intracapsular = proximal to intertrochanteric line; retinacular arteries (branches of medial/lateral circumflex femoral from profunda femoris) travel retrograde up the neck - displacement disrupts them → AVN risk
- Extracapsular (intertrochanteric/subtrochanteric) = blood supply intact, no AVN risk
AVN risk is unique to intracapsular displaced fractures - this single fact drives all surgical decision-making (fixation vs arthroplasty).
