- More common in men; peak incidence fifth and sixth decades for primary disease
- Primary (idiopathic) cause: IgG antibodies against PLA2R (phospholipase A2 receptor) on podocytes - forms subepithelial immune complexes → complement activation via MAC → podocyte injury
- Carries the highest thromboembolic risk of all nephrotic glomerulonephritides
