Breast abscess

Key high-yield points
- First-line (no antibiotic indication): continue breastfeeding (or express if too painful) + simple analgesia + warm compresses
- Antibiotics indicated if: infected nipple fissure present, symptoms not improving after 12-24 hours despite effective milk removal, OR breast milk culture positive
- First-line antibiotic: flucloxacillin 10-14 days (lactational; *Staph aureus* cover)
- Penicillin allergy: erythromycin or clarithromycin
Do NOT stop breastfeeding - milk stasis is the initiating factor for lactational mastitis. Stopping breastfeeding abruptly worsens stasis and delays resolution. Milk from the affected breast is safe for the infant during antibiotic treatment.
