  • The single exam question tests recognition of intestinal malrotation with volvulus and its definitive management. Key differentials in the paediatric setting:

Bilious vomiting in a neonate = upper GI contrast study to exclude malrotation. If intestinal rotation confirmed → Ladd's procedure. If signs of vascular compromise → urgent laparotomy.

Passing meconium makes Hirschsprung disease unlikely. Malrotation can affect any age but classically presents in the first few days of life with bilious vomiting.
