Kawasaki disease

Key high-yield points
- Medium-vessel vasculitis predominantly affecting children under 5. Diagnosis requires fever ≥5 days plus at least 4 of 5 CREAM features.
- Conjunctivitis - bilateral, non-exudative (no discharge)
- Rash - polymorphous (maculopapular/urticarial), widespread; never vesicular
- Erythema/oedema of hands and feet - progresses to periungual desquamation in subacute phase
- Adenopathy - unilateral cervical lymphadenopathy, nodes typically >1.5 cm
- Mucosal changes - dry cracked lips, strawberry tongue
Child appears disproportionately unwell and irritable - more toxic than expected for a viral illness. Fever is typically unresponsive to antipyretics.
