- Location - pressure points: plantar 1st/5th metatarsophalangeal joint, heel, under calluses
- Painless - cardinal feature; patient may be unaware of ulcer entirely
- Punched-out appearance with deep sinus tract
- Surrounding callus - often overlies or conceals the ulcer
- Warm foot with palpable pulses - if arterial supply intact (distinguishes from arterial ulcer)
- Brisk bleeding on probing/debridement (unlike arterial ulcers)
- Dry, fissured skin from autonomic dysfunction (anhidrosis)
