  • Prodrome (1-5 days): low-grade fever, malaise, mild upper respiratory symptoms
  • Lymphadenopathy - suboccipital, post-auricular, posterior cervical; appears *before* rash - most characteristic feature
  • Rash - pink-red maculopapular, starts face, spreads cephalocaudally; fades within 3-5 days
  • Forchheimer spots - petechial lesions on soft palate; classic but non-specific
  • Arthralgia/arthritis - especially adult women, small joints of hands and wrists
  • Up to 50% of infections are subclinical
  • Infectious period: 7 days before to 4 days after rash onset
