Sebaceous hyperplasia

  • Small (1-5 mm), dome-shaped, soft, yellow/pale yellow-white papule - reflects lipid-rich sebocytes
  • Central umbilication (dell) - defining feature; central pore corresponding to dilated follicular infundibulum
  • Lobulated surface - multilobulated, cauliflower-like, yellow lobules around the central dell
  • Distribution - sun-exposed face: forehead, nose, cheeks, temples
  • Asymptomatic - no itch, spontaneous bleeding, or ulceration; if present, reassess
  • Multiple lesions - consider ciclosporin therapy or Muir-Torre syndrome

Dermoscopy shows a crown of vessels (hairpin/linear) around a central follicular opening with pale yellow lobules - highly characteristic and helps exclude BCC.
