Crohn's disease

  • Right iliac fossa pain - colicky, due to terminal ileal involvement (most common site, ~70%)
  • Diarrhoea - chronic, may be nocturnal; less bloody than UC
  • Weight loss and malnutrition - malabsorption, reduced intake, increased GI losses
  • Perianal disease - skin tags, fissures, fistulae, abscesses
  • Extra-intestinal manifestations - occur in >20%, especially with colonic disease
  • Joints - peripheral arthropathy (mirrors gut activity), sacroiliitis (independent)
  • Eyes - episcleritis (correlates with activity), uveitis (can be independent)
  • Skin - erythema nodosum (mirrors activity), pyoderma gangrenosum (independent)
