Gestational hypertension

Key high-yield points

  • New-onset hypertension (BP ≥140/90 mmHg) after 20 weeks gestation
  • No proteinuria and no end-organ dysfunction - if either present, diagnosis is pre-eclampsia
  • Hypertension before 20 weeks = pre-existing (chronic) hypertension, not gestational

Proteinuria ≥1+ on dipstick or PCR ≥30 mg/mmol = pre-eclampsia, not gestational hypertension. This distinction is tested repeatedly.
