Pre-eclampsia

Key high-yield points
- Pre-eclampsia - new-onset hypertension (BP ≥140/90 mmHg) after 20 weeks + significant proteinuria (PCR ≥30 mg/mmol or ≥1+ dipstick) OR evidence of end-organ dysfunction
- Gestational hypertension - new-onset hypertension after 20 weeks WITHOUT proteinuria or organ dysfunction
- Hypertension before 20 weeks = pre-existing (chronic) hypertension, not pre-eclampsia
Pre-eclampsia is often asymptomatic and detected only at routine antenatal checks - do not be reassured by an asymptomatic presentation. A woman with BP ≥140/90 + ≥1+ proteinuria meets diagnostic criteria even if she feels completely well.
