Key high-yield points
- Hypoparathyroidism: absent PTH → reduced osteoclast activity + reduced renal Ca2+ reabsorption + reduced renal PO4 excretion + reduced calcitriol synthesis → ↓ Ca2+, ↑ PO4, ↓/undetectable PTH, normal/low ALP.
Low Ca2+ + raised PO4 + low PTH + normal ALP = hypoparathyroidism. The raised phosphate distinguishes it from osteomalacia (where PO4 is low). ALP is not elevated because bone turnover is reduced, not increased.
