Nappy candidiasis

Key high-yield points
- Satellite lesions - small erythematous papules/pustules beyond the edge of the main rash; most characteristic feature
- Skin fold involvement - extends into inguinal creases and perianal folds (contrast with irritant dermatitis which spares folds)
- Bright erythema - vivid 'beefy' red with glazed appearance
- Failure to improve with barrier cream alone - key clue
Satellite lesions + skin fold involvement = candidal nappy rash. Irritant nappy rash has NO satellite lesions and SPARES the skin folds.
